Compulsory maternity application (LEAVE AND ALLOWANCE)

During the period of leave , the mother will be entitled to receive remuneration: the maternity allowance is equal to 80% of the last daily salary. It is the employer or the INPS who pays, as in the case of cleaners and carers.

What documents are required to apply for maternity benefit?

The following information from the registrant is required to apply for maternity benefit:

• Identity card and tax code of the applicant
• Paycheck
• Medical certificate of pregnancy
• Newborn/newborn birth certificate
• INPS SR163 model signed and authenticated by the bank or post office official (available after purchase to be completed and attached to the request)
• Model INPS SR14 or Model SR01 (available after purchase to be filled in and attached to the request)
• Assistance and representation mandate (available after purchase to be filled in and attached to the request)
